Article Title: Performance of Acoustic Radiation Force Impulse Ultrasound Imaging for Carotid Plaque Characterization with Histological Validation

doi: 10.1016/j.jvs.2017.04.043

Figure Lengend Snippet: Patient characteristics.

Article Snippet: In Vivo ARFI Ultrasound Imaging ARFI imaging was performed with the carotid artery in longitudinal orientation using a Siemens Acuson Antares (Siemens Medical Solutions USA Inc., Ultrasound Division) and a VF7-3 linear array 16 .

Techniques:

In vivo ARFI image of the internal carotid artery from a symptomatic patient with matching micro-CT and histologic sections. B-mode (a) and ARFI peak displacement (b) images depict a significant stenosis due to plaque in both proximal and distal walls (white outline). Segmentations from a blinded reader (c) identify areas of increased peak displacement (indicating necrotic core, NC) in both the proximal and distal walls, covered by areas of low peak displacement (indicating fibrous cap, FC). The rest of the plaque has low displacement (indicating collagen deposition, COL). Micro-CT image and H&E and CME sections show an AHA type Va plaque with NC (red outline) and FC (yellow outline) in both proximal and distal walls (d). Calcification is observed in the distal wall (d, dashed magenta outline) coincident with the NC.

Comparison of average fibrous cap thickness measurements from ARFI versus histology for the three blinded readers. Linear regression and correlation analysis is shown in left column, with Bland-Altman plots in right column. RPC, coefficient of reproducibility; CV, coefficient of variation; SD, standard deviation; rho, Spearman correlation coefficient; R2, coefficient of determination. MATLAB plotting code developed by Ran Klein.
